🩺 Role Responsibilities:

• Deliver skilled nursing care in accordance with an active plan of care
• Administer medications, treatments, and therapies as prescribed
• Accurately document assessments, activities, and patient responses
• Educate patients and families on care, self-management, and disease processes
• Collaborate with physicians and clinical teams to provide the highest quality care

🎯 Requirements:

• Active Washington or Compact LPN License (Required)
• Strong attention to detail and organizational skills
• Effective communication and time management
• Ability to solve problems and resolve conflicts independently
• Previous home health experience preferred , but not mandatory
