Physical Therapist at Heartland Home Health summary:

A licensed Physical Therapist delivers skilled therapy services to patients with injuries, illnesses, or disabilities to help them reach optimal physical function. They collaborate with physicians and clinical teams to develop, implement, and adjust customized treatment and rehabilitation programs. This role requires a Master’s degree, state licensure, CPR certification, and experience in hospital or home health settings.

Overview:

We are looking for a Physical Therapist to join our team. The Licensed Physical Therapist assumes responsibility and accountability for the delivery of appropriate physical therapy services within a framework of interdisciplinary collaboration and consultation.

  • Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function.
  • Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
  • Develop/implement a conditioning/rehabilitation program consistent with the physicians' Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
About You:
  • Master’s degree from an accredited Physical Therapy Program (approved by the
  • American Physical Therapy Association, the Committee on Allied Health Education and
  • Accreditation of the American Physical Therapy Association)
  • PT Licensure in the state of practice
  • Current CPR Certification
  • One year's experience in hospital or home health is preferred
